LIABILITY WAIVER I recognize that certain hazards and dangers are inherent in the events and programs of CrossWay Community Church. I acknowledge that although CrossWay Community church has taken safety measures in minimizing the risk of injury to participants, CrossWay community Church cannot guarantee that the participant's equipment premises, and/or activities will be free from hazards, accidents and /or injuries.

In consideration of CrossWay community Church accepting and permitting my child to participate, I agree that CrossWay Community Church, a non-profit corporation, its agents, officers, employees, trustees, and volunteers will not be liable for any injury, death damage and/or loss to my child, and/or anyone claiming on my child's behalf, and I further agree to hold harmless, indemnify and defend CrossWay Community church, its officers, agents, employees, trustees, and volunteers for an from any and all damage, whether such injury, illness or damage occurs on or off the church premises. 

PHOTO RELEASE I certify that photographs or videotape picture of my child participating in CrossWay Community church programs may be reproduced and utilized in promotional materials for the Church such as social media pages. Please initial below if you would not like your child's photograph or videotape pictures used for promotional materials.

I, the undersigned parent or guardian of  list name below  a minor, do hereby authorize adult worker with CrossWay Community church to act on my behalf in the vent that the above child requires medical treatment, and I cannot be reached at the phone numbers provided above. 

Further, as parent or guardians of the minor named above, I do hereby expressly consent that my son/daughter may receive emergency medical treatment from any physician, hospital, or another medical center without the necessity of first notifying me. and do further agree to hold blameless any physician, hospital or other medical center for rendering such services. In addition, I will hold blameless the Northeast District of the Wesleyan Church, the above church and other sponsors involved in the activity.
